The Evolution of Biomarkers of Magnetic Resonance Enterography to Quantify Inflammation in Crohn's Disease

Although fibrocolonoscopy with targeted biopsy is currently considered the “gold standard” in Crohn's disease's diagnosing, there are always factors that limit this diagnostic method (significant length of the small intestine, the large number of bends and loops, segmental lesions of the mucosa). MR enterography - a minimally invasive diagnostic study that allows you to visualize the internal structure of the intestine, is absolutely safe technology without radiation exposure. The use of scales of activity of inflammation in Crohn's disease allows to unify the array of biomarkers of the disease and objectify the results of patient monitoring in the dynamics of assessing the severity and degree of activity of inflammation of the affected intestinal segment in Crohn's disease. The following indices are used: MaRIA (Magnetic Resonance Index of Activity, 2002), Lemann (2011), CDMI (Crohn's Disease Magnetic Index, 2012), Clermont score (2013), MEGS (the Magnetic Resonance Enterography Global Score, 2014). An arsenal of scale ratings was developed through comparison with clinical and endoscopic biomarkers of activity and showed a high correlation. A universal scale does not exist at this time, and each of the available has its own drawbacks, so the modification of the existing and the development of new scales continues.
