Survey of Patients with Chronic Obstructive Pulmonary Disease Based on Cloud Services

The electronic questionnaire for patients with chronic obstructive pulmonary disease (COPD) is considered. The paper offers solutions for the translation of COPD assessment test (COPD Assessment Test, CAT) on electronic documentation. Since the test is supposed to be used in closed networks of medical institutions and in home networks of users (with Internet access), the electronic test is developed in different systems: MS Excel, Google Forms, Google Sheets. The main problems in the electronic survey of patients are considered and the ways of their solution are offered. The survey form is adapted to small smartphone displays. Three survey forms have been developed. The first form allows you to fill out the test on a local computer with Windows without access to the Internet. The second form can run on any operating system, but requires the user to have a Google account and connection to the Internet. The third form can be run on any operating system, does not require an account in Google, but has limited functionality.
