It looked the same to me

The crop cars and sump pumps at a steel plant casting facility had to be locked out for cleaning of the crop pit. There is a complex lockout procedure involving the crop car and two sump pumps along with a 120 volt control system. After half of the cleaning was completed, the system was unlocked and the cars moved to complete the cleaning. The system was then relocked. During the process of relocking the system, the wrong 120 volt control circuit breaker was locked out. There are two motor control centers side by side with essentially the same design that looked identical. In the process of locking out the 120 volt control circuit breaker for the sump pumps, the 120 volt breaker was locked out in the other motor control center. This is a case history on the wrong circuit breaker being locked out due to look-a-like equipment and human error as a result of complacency. It will also cover the counter measures put in place to prevent future incidents.
