A serious near-miss when disconnecting a 4 kV motor

This paper describes a serious near-miss when two workers isolated a 4 kV motor for maintenance work. The near-miss had the potential for serious injuries or fatalities. This paper also describes what actions including new and unique measures were taken to eliminate the re-occurrence.
