An innovative air accident investigation method to safety differently

Flight safety is a paramount for aviation operations; operators, organizations, agencies and authorities are always seeking the state-of-the-art methods, procedures, requirements and processes to reach the safety objectives and standards. The probability of air accidents is significantly low, in line with the number of miles flown and the number of passengers carried. Statistical data, as presented in IATA and ICAO reports, indicate that technological safety in aviation has reached its asymptotic limit, which is a cause for concern given the predicted increase in the number of accidents in the future. The technical reports indicate that, over recent years, there has been a reduction in accidents attributed to technical factors, while accidents with the human factor as the root cause have increased. Consequently, the investigative sector was obliged to adapt to this change by employing increasingly sophisticated analytical techniques and methods. The model proposed is a new accident model, defined as a hybrid. The new hybrid model helps to unravel the mishap causes and it has as its strength the passage through various analyses. To support the proposed model, studies have been conducted on four accident causes that have the human factor as root cause. The proposed model emphasized that everything starts among the organizational system. Furthermore, it came out that training represents a key component of the incident reduction process, enabling the identification and appropriate response to the hazards. Since the human factor that cannot be eliminated, the new method will have limitations, but these can be overcome thanks to the synergetic collaboration of the various described approaches. The work is also intended as a prospective analysis aimed at prevention and preparation for future scenarios, given that it is well known that in the field of technology, we should be able to accept change and find new solutions.
