Non-Invasive Assessment for Dynamic Cerebral Autoregulation Based on Near-Field Coupling

Dynamical cerebral autoregulation (dCA) plays a pivotal role in the clinical assessment of strokes. In this study, we utilized the Near-Field Coupling (NFC) principle to monitor cerebral blood flow (CBF) while simultaneously synchronizing the monitoring of mean arterial pressure (MAP). We conducted experiments involving changes in body position with volunteers to study the feasibility of the device in assessing dCA. To validate the feasibility of monitoring dCA with the NFC device, we compared the CBF and dCA indices obtained through Transcranial Doppler (TCD) with those measured by the NFC device. Additionally, a Bland-Altman consistency analysis was performed for dCA indices. To further confirm the device's capability to reflect differences in dCA at various levels, changes in body position at different angles were induced for volunteers, and statistical analysis was conducted on the time required for dCA to return to the initial level. In the postural change experiment, we capture CBF waveforms and MAP. By comparing with TCD, we validated the feasibility of the NFC principle in monitoring CBF. The Bland-Altman analysis of dCA indices RFx and rCVR indicates that the majority of measurement differences fall within the 95% confidence interval. There is good consistency between NFC and TCD in assessing dCA, indicating the practicality of NFC in monitoring dCA. In experiments involving changes in body position at different angles, adjustment times were 0.6, 0.8, and 1.2 minutes, respectively, suggesting the potential of NFC to differentiate regulatory levels. This study has provided a new approach for assessing dCA.
