Melding System Safety Methodologies for a Structured and Comprehensive Risk Assessment of Optionally Manned Aircraft

In a context where safety can be challenged due to a weakened organizational layer, the need for early and high-quality hazard identification is heightened. This paper advocates for the combination of several mature safety methods for hazard identification to achieve the overarching objective of comprehensive hazard determination. Key characteristics supporting this objective are defined and described for each of the methods. This allows to develop a scheme to achieve higher benefits from their overlap, while compensating for any of the weaknesses. In the specific case of optionally piloted aircraft, the combination supports a seamless transition between human pilot and automation, when it comes to identify hazard causal factors. This paper introduces a Hybrid Hazard Identification Process (HHIP).
