The tendency to hypercoagulation in the perioperative period of uterine artery embolization

Purpose of the study: to assess the dynamics of changes in the blood coagulation system in the perioperative period during uterine artery embolization (UAE) based on monitoring of hemostasis system's indicators, including TEG, and the tendency (or its absence) to thrombosis. The study included 37 patients with EMA, who in the course of preoperative examination excluded the contributing risk factors for thromboembolic complications. In the perioperative period, dynamic monitoring of coagulogram and thromboelastography was performed. One day after the UAE, the patients showed a tendency to hypercoagulation according to the TEG data (a decrease in the R, K time indices and an increase in the α-angle, MA value) and fibrinolysis activation (increased LY 30 index). At the same time, in the study of coagulogram indices, an increased level of hypercoagulation markers (fibrinogen, D-dimer, SFMC), Prothrombin index and a decreased APTT were revealed. Findings indicate, in general, an increased risk of VTE complications in the postoperative period after UAE. The revealed features of changes in hemostasis parameters in patients after UAE indicate the need for continuous monitoring of the hemostasis system and the development of effective measures for the prevention of VTE complication.
