SMaRT-PD CDSS: A Service Improvement Initiative for Self-Management and Clinical Decision Support in Parkinson’s Disease - Development and Initial Validation.

We created Home Based Care (HBC), an innovative care pathway offering an alternative to standard care for People with Parkinson’s (PwP). Despite its success, scalability is limited by resource inefficiencies and manual processes. Clinical Decision Support Systems (CDSS) are digital health tools designed to provide tailored, evidencebased support at the point-of-care, especially in underserved areas. In this paper, we present an overview of the initial validation of SMaRT-PD, a CDSS designed to automate key HBC decision-making processes. Our aim was to evaluate the accuracy of this tool, and to identify gaps and conflicts by utilising a curated dataset as part of a service improvement initiative. Over 1900 separate Rule-Based Decision Trees (RBDT) tests were conducted using data from over 200 PwP. SMaRT-PD outputs were compared with the outputs generated by expert clinical review of the datasets. Our preliminary results demonstrate that SMaRT-PD accurately represents clinical knowledge and supports automated clinical decision-making, enhancing its potential for scalable Parkinson’s care.
