SafeMeds - Revolutionising Medication Reconciliation for Elderly Care with Digital Innovation

The complicated issue of polypharmacy, which causes medication-related problems and negative drug reactions, currently plagues Malaysia's aging population. Pharmacists regu-larly fail to devote sufficient time to medication reconciliation. In response, this paper introduces SafeMeds, a digital application in-tended to streamline medication reconciliation for elderly patients in Malaysian public hospitals. SafeMeds addresses challenges such as high pharmacist workloads and system integration issues, enhancing patient safety and alleviating workload burdens. Leveraging advanced technologies such as machine learning, and adhering to healthcare standards, SafeMeds offers a user-friendly interface and real-time alerts for drug interactions. SafeMeds aims to contribute towards Goal 3 of the UN Sustainable Development initiative of ensuring healthy lives and promoting well-being at all ages.
