Web-based Platform for Cardiovascular Diseases Associated Risk Assessment and Personalized Remote Patient Monitoring

This paper presents a web-based digital platform for remote web-based platform for cardiovascular diseases associated risk assessment and personalized remote patient monitoring. Built on the extended Systematic Coronary Risk Evaluation algorithm (SCORE2), it offers personalized assessments, recommendations, and automated real-time alerts for healthcare providers. The platform features a comprehensive questionnaire, BMI calculations, and a digital patient diary for tracking related health metrics. It facilitates virtual consultations, enhances patient compliance, and supports proactive, personalized care by integrating medical information technology (MIT) to monitor dynamic changes in patients’ health, thereby improving early risk identification a nd disease prevention. The multi-stage validation process of the cardiovascular disease management tool demonstrated its effectiveness i n a real-world setting, with positive feedback from both patients and clinicians. Patients appreciated its ease of use, while cardiologists valued its ability to increase patient awareness, support clinical decision-making, and streamline consultations. Recommendations for future enhancements include refining t he u ser i nterface, a utomating d ata reviews, and optimizing notifications ystems. The tool’s p otential to reduce CVD burden, improve healthcare efficiency, and enhance patient understanding highlights its significance $\boldsymbol{i} \mathbf{n}$ advancing cardiovascular disease prevention and management, particularly in underserved populations.
