Coupling Multi-factor Hazard Analysis Based on HAZOP and DEMATEL

The mission failures or accidents of a complex industrial systems usually result from the mutual coupling of multiple factors. However, the widely used methods for hazard analysis such as Hazard and Operability Analysis (HAZOP) mainly focus on a single factor from a qualitative perspective. On the other hand, the Decision Making Trial and Evaluation Laboratory (DEMATEL) technique is an effective approach for determining the direct or indirect relationships among all components of a system, and can support the quantitative assessment of the complex relationships of the risk factors underlying a failure or an accident. Based on the HAZOP method, this paper evaluates the impact of coupling multifactors on the safety of a system from both qualitative and quantitative perspectives with the combination of the DEMATEL technique. A case study of the take-off process of carrier-based aircraft is reported. The study resulted in identification of key risk factors, and recommendations of risk mitigations.
