Through the introduction of balanced scorecard, on the basis of its financial, customer, internal operation, learning and development to improved the balanced scorecard, this paper highlighted "green" and increased social environmental factors and perfected performance evaluation index of green supply chain. Further, it put forward the five-level indicators and 17 secondary indicators which constitute performance evaluation index system of green supply chain management, designed a performance evaluation model of green supply chain. It provided a reference for enterprise's performance evaluation of green supply chain and has a certain theoretical and practical significance.
